Advanced Practice: Beyond the Corneal Abrasion

CORNEAL ULCER

Diagnosis

Utmost importance to determine cause of corneal ulcer (i.e. prolonged contact lense use, trauma, foreign bodies, chemicals, etc) and remove offending agents

Typically round, white lesions that can be identified by the naked eye with pen light but can also be visualized on fluorescein staining. Sometimes may be larger lesions.

Treatment

Remove offending agent

Broad spectrum topical antibiotic application +/- microbiological investigation

  • Typically q 1 hour antibiotics drops

Optho consult and close follow up

Importance

Can lead to corneal scarring causing visual impairment and even blindness!
