Monday: Back to Basics - Omphalitis
Typically superficial cellulitis, however can travel via ophalomesenteric duct and lead to systemic infection
IV antibiotics: Vancomycin or Gent PLUS Clinda or metronidazole
Surgicial consultation for possible debridgement
Tuesday: Advanced Practice - Refractory VT
Treatment Escalation Pathway:
Amiodarone -> Beta Blockade (nonselective may be better the cardioselective)
Propofol (also for sedation for cardioversion but can terminate electrical storm) -> Consult Electrophysiology (Cardiology)
Wednesday: What's the Diagnosis?
Irregular tachycardia with varible QRS morphology?
Could it be Afib in a patient with RVR?
What medication should you use? Procainamide. If that fails, then what?
Saturday: Board Review - Meningitis
